Electronnic Logistic Management Information System (eLMIS) Upgrading
I. Background:
In Vietnam, as PEPFAR is transitioning out of Direct Service Delivery support, it is time for the country to take ownership of financing and managing HIV/AIDS programs. The Social Health Insurance (SHI) fund, managed by Vietnam Social Security (VSS), is the single most important financing mechanism for the sustainable HIV response. According to VAAC’s plan, 160,000 out of approximately 171,000 patients currently on ARVs received ARVs through the SHI scheme in 2023. This situation requires more detailed and consistent monitoring and management of HIV patients under the SHI system, thereby improving data accuracy, comprehensiveness, and timeliness. However, when PLHIV receives medical services through the SHI scheme, the VSS e-claim system currently captures only information as with those of other diseases but not specific information for HIV treatment program management. Meanwhile, each health facility uses its own Health Information System (HIS) to collect this information. Thus, there are substantial variabilities in the name, definition, and calculation of ARV treatment-related variables across health facilities.
To address the barrier, the establishment of standardized, common HIV treatment management indicators/variables to be integrated into the VSS e-claim to monitor HIV patients under the SHI scheme are urgently needed.
During 2021 and 2022, LHSS worked with DHI and VAAC to revise Decision 4210 on standard data outputs in management, verification, and payment for medical services covered by the SHI fund to include HIV treatment indicators. As a result, on January 18, 2023, MOH enacted Decision 130 on output data standards for management, verification, and payment for medical services covered by the SHI fund, including HIV data standard outputs, that replaces Decision 4210.
From September 1, 2023, onward, all health medical and examination facilities will send treatment data (Extensible Markup Language - XML) based on information structures in decision 130/Q?-BYT to an insurance information gateway at https://gdbhyt.baohiemxahoi.gov.vn.
So far, the gateway of the VAAC has not yet been ready to receive the data from the facilities. On the other hand, the eLMIS system, which is currently maintained and under management by VSS, is proven a strong ability to consolidate and export HIV data from the e-claim system for program management purposes in an effective way. Therefore, as part of the support for the government in the implementation of Decision 130, LHSS plan to support VSS in developing a function to retrieve and store the database of the HIV patient treatment program management indicators following Decision 130 that integrate into the current eLMIS system, while the VAAC gateway is not ready to receive these databases. LHSS project will seek an IT company to conduct this assignment..
