PATH is a global nonprofit dedicated to a mission to advance health equity through innovation and partnerships. To enable early access to healthcare services at community, and to effectively detect, control, and manage hypertension and diabetes within the community, the project "Transforming Responses to Non-Communicable Diseases in Vietnam" has been implemented by PATH in collaboration with the Department of Preventive Medicine in some provinces and cities.
With the message "For a Healthy Vietnamese Community," the project team has developed the "Song khoe" application. With smartphones, individuals can simply download the "Song khoe" application to their devices to help them self-screen and assess the risk of CMDs (hypertension, diabetes).
In 2024, PATH will continue to mobilize resources to update and enhance the applications for screening, risk assessment, treatment management, and support for the care of CMDs, including hypertension, diabetes, dyslipidemia, and obesity. The app will be implementing in Thai Nguyen province. The detailed RFP#2024-Amgen-01 attached provides the update and upgrade requirements to meet the project's needs in the upcoming period.
